Patient Information

Appointments can be made by contacting the surgery during office hours on 3394 1071 or 3397 3331.

What to bring to your first appointment

It is a government requirement that you obtain and bring with you a current referral from your referring doctor. GP referrals last for 12 months, while referrals from other specialists last 3 months.

We will need details of your current Medicare card and your private health insurance policy number.

If possible please bring along reports and/or copies (if available) of any recent tests that have been done e.g. scans, X-rays, blood tests etc.

If you are attending for your first antenatal appointment, it is ideal to have a fairly full bladder so that the doctor can do a small scan.

If you are unable to keep your appointment

If for any reason you cannot make your scheduled appointment, it would be greatly appreciated if you could call and advise us as soon as possible, so that we can offer your appointment to another patient.

Your Contact Information

Please ensure when you make your appointment that we have a current contact number on which we can reach you at all times.

Unfortunately given the nature of this business, the doctor may be required to leave the practice to attend a delivery or emergency operation which means patient appointments need to be rescheduled. Having the correct contact number will minimise the inconvenience to you.

We're here to help

Our friendly reception staff are available to answer any other queries you may have so please don't hesitate to call.
